Use this organiser to record the questions, evidence and specialist decisions for a particular product and market. It is not an official form or a universally complete launch checklist. Use the version and review information shown on this page with the files.

The pack contains separate Ireland/EU, Great Britain and Northern Ireland CSVs. Each uses the same administrative columns, while its market and source context remain distinct. CSV files can be opened in common spreadsheet applications. Do not merge the market tabs into one assumed legal outcome.

What the columns are for

• Product reference and revision identify the record under consideration.
• Market, role question and task identify what needs to be scoped.
• Primary source and provision identify the basis the specialist must check.
• Evidence reference, presence and completeness keep document states distinct.
• Reviewer, decision date and findings record the actual review.
• Owner, next action and due date make follow-through visible.

Use the blank file with the synthetic example

Save a working copy for the real product. Identify the scope before entering evidence. Add known information, keep missing facts visible and ask the specialist to confirm the relevant tasks. A blank row is a place to record work, not evidence that all legal obligations have been listed.

The synthetic example deliberately contains unresolved role and evidence questions for Sample Product A. It includes no approval or safety conclusion. Its dates, names and record references must not be copied into a real product file as though they concern that product.

Review and maintain the record

Record the exact version reviewed and the meaning of each decision. Revisit the affected rows when a product, source or supply arrangement changes. Keep old findings as history and identify what needs a new decision.

Keep the blank file, synthetic example, field instructions and page review details together. The reviewed resource helps organise the task; it does not certify the individual product or establish that every applicable obligation has been listed.
